Find the Mic Sweepstakes 2011 - Rules
To enter the monthly sweepstakes, send an e-mail message to firstname.lastname@example.org or send a letter to Radio magazine; 5285 Shawnee Rd, #100; Alexandria, VA 22312.
Tell us which issue you are submitting an entry for, where the mic icon is hidden on the cover of the issue, your name, job title, company name, address, telephone number and e-mail address.
Failure to include the required contact information will disqualify your entry.
What you can win
Each month, a winner will be randomly chosen from the correct entries. The prizes for 2011:

The official rules follow.

The sponsors ("Sponsors") of this sweepstakes are NewBay Media LLC., d/b/a Radio magazine, at 5285 Shawnee Rd, #100, Alexandria, VA 22312; and Hosa Technology, 6650 Caballero Blvd., Buena Park, CA 90620.
How to Enter:
Qualified subscribers of Radio magazine (see Eligibility Rules below) may enter by sending an e-mail message or typing or hand-writing the following information on your company letterhead or a standard 8-1/2" by 11" sheet of paper:
1. Your name, job title, company name, address, telephone number and e-mail address.
2. A complete and accurate description of the location of the mic icon on the cover of the most recent 2011 monthly issue of Radio magazine. Be as specific as possible in describing the location of the necessary mic icon. Note: The icon is not the "i" in the Radio magazine logo. If you do not have the necessary issue, you may obtain a copy of the cover of an issue by sending a request and self-addressed stamped envelope to Radio magazine Cover Request, 5285 Shawnee Rd, #100; Alexandria, VA 22312.
3. E-mail the information to email@example.com, or mail your entry to Radio magazine Find the Mic Sweepstakes, 5285 Shawnee Rd, #100; Alexandria, VA 22312, or fax it to (888) 386-4548.
4. All entries (mail and online) must be received no later than 5 p.m. Central Time on the 10th day of the month following the cover date of the issue referenced in the entry. Multiple entries are not permitted. One entry per person per mailing address and per e-mail address per month. Sponsors are not responsible for late, lost, damaged or misdirected mail, faxes or e-mail and timely submission of entries is the sole responsibility of entrants. Submitted entries will not be returned and become the property of Sponsors.
Winner Selection and Notification:
A monthly random drawing will be conducted from all the entries correctly describing the location of the mic icon by the 11th day of each subsequent month. The Sponsors will use reasonable efforts to contact each monthly winner by the 15th day of that subsequent month and will be announced in the issue two months after the cover date of the entry.
Each drawing will be conducted by the Sponsors. All participants agree to be bound by these rules and the decision of the Sponsors, whose decisions are final and binding in all matters relating to the Sweepstakes.
Odds of winning depend on the number of correct, eligible and legible entries received each month.
Each monthly winner will receive one of the following prizes on the month:

Total approximate retail value of all prizes is $879.40. Sponsors are not responsible for lost or mutilated prizes and none will be replaced.
Substitution of prizes will not be permitted except by Sponsors who reserve the right to substitute a prize of equal or greater value if indicated prizes are not available. All taxes, fees and surcharges on prizes are the sole responsibility of the winners.
Sweepstakes open to qualified subscribers of Radio magazine who are residents of the 50 United States and the District of Columbia and who are eighteen (18) years of age as of January 1, 2011, and excludes the employees and immediate family (spouses and parents, siblings, children and each of their spouses) of Sponsors, the prize manufacturers, and their parents, affiliates, subsidiaries, advertising agencies and any other company involved with the design, production, or execution of the sweepstakes. Also excluded are any individuals domiciled with any of the individuals excluded above. Qualified subscribers are determined in the sole discretion of Radio magazine. There is no charge for a subscription to Radio magazine to qualified subscribers located in the United States.
Sweepstakes begins at 12:01 a.m. Central Time on January 1, 2011, and ends at 5:00 p.m. Central Time on December 31, 2011.
WINNERS RELEASE THE SPONSORS, THE PRIZE MANUFACTURERS AND EACH OF THEIR PARENTS, AFFILIATES, OFFICERS, AGENTS AND EMPLOYEES FROM ANY RESPONSIBILITY OR LIABILITY IN CONNECTION WITH ANY LOSS, ACCIDENT, OR DEATH INCURRED IN CONNECTION WITH THE USE OF OR THE INSTALLATION OF THE PRIZES WON IN THE SWEEPSTAKES. The winners hereby consent to the use of his or her name and/or likeness by the Sponsors for advertising purposes without additional compensation unless prohibited by law. The verified winners may be required to sign an Affidavit of Eligibility and a Publicity/Liability Release. Any requested materials, including, but not limited to said releases, must be returned within ten (10) days of the date of notification. Failure to comply within any request in the allotted time frame will result in disqualification and the selection of an alternate winner. All taxes on the prizes are the sole responsibility of the winner. All federal, state, local, municipal and provincial laws and regulations apply.
By participating, entrants acknowledge and agree to be bound by these rules, and the decisions of the Judges, which are final. Sponsors do not make and are not responsible for any warranty (including fitness for particular purpose) or guarantee with regard to any prize or portion thereof.

Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operations or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of entries. The Sponsors are not responsible for any problems or technical malfunction of any telephone network or lines, computer online systems, servers or providers, computer equipment, software failure of any e-mail or entry to be received by Sponsor or congestion on the Internet or at any website, any combination thereof or otherwise, including any injury or damage to entrants or any other person’s computer related to or resulting from participation or downloading any materials in the Sweepstakes.
In the event of a dispute, entries made by Internet will be deemed made by the authorized account holder of the e-mail address submitted at the time of entry. The "authorized account holder" is deemed the natural person who is assigned to an e-mail address by an Internet access provider, service provider or other online organization that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. A potential winner may be requested to provide Sponsors with proof that the potential winner is the authorized account holder of the e-mail address associated with the winning entry. If for any reason the Sweepstakes is not capable of running as planned, including due to infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failure, human error or any other causes beyond the control of Sponsor that corrupt or affect the administration, security, fairness, integrity, or proper conduct of the Sweepstakes, Sponsors reserve the right in their sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend the Sweepstakes. Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries. The Sponsors are not responsible for any problems or technical malfunction of any telephone network or lines, computer online systems, servers, or providers, computer equipment, software, failure of any e-mail or entry to be received by Sponsors on account of technical problems or traffic congestion on the Internet or at any website, any combination thereof, or otherwise, including any injury or damage to entrant's or any other person's computer related to or resulting from participation or downloading any materials in the Sweepstakes. Sponsors are not responsible for late, lost, illegible, incomplete, stolen, or misdirected entries. Caution: Any attempt by an entrant to deliberately damage any website or undermine the legitimate operation of the sweepstakes may be a violation of criminal and civil laws and should such an attempt be made, sponsors reserve the right to seek damages from any such person to the fullest extent permitted by law.
